Beer and Nitroglycerin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

VASODILATORS

Why this applies to Nitroglycerin

Nitroglycerin falls under VASODILATORS — the group Beer interacts with.

Concomitant use may increase the risk of acute hypotension.
Acute alcohol intoxication can increase the risk of hypotension and additive effects with vasodilators.
Severity Moderate Likelihood Probable Evidence C

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know if you use nitroglycerin and enjoy a beer: both work to widen and relax your blood vessels. Nitroglycerin does this on purpose to ease the heart's workload, and alcohol does it too as a side effect. Stacked together, they can drop your blood pressure more than either one alone.

In real life, that combination can leave you feeling dizzy, lightheaded, flushed, or even faint, especially when you stand up quickly. The risk is highest with larger amounts of beer. This is a well-recognized, moderate interaction, not a rare or theoretical one. I wouldn't change anything on your own, but please check with your pharmacist or doctor about whether any beer is okay for you.

From our pharmacist’s overview on the Nitroglycerin page

“Nitroglycerin (Nitrostat, Nitro-Bid) exists to open blood vessels quickly during angina, so a supplement that also drops blood pressure, or one that fights the drug's effect, is not a small matter, and this list warrants real care before you add to it. The most serious entries pull in two directions.”

About Beer

What is Beer?

Beer
  • Social and recreational drinking
  • Relaxation
  • Heart health (popular belief)
  • Bone health (popular belief)
  • Source of antioxidants and B vitamins

Beer is an alcoholic beverage that contains some minerals, B vitamins, and plant compounds from hops and grain, but it is not a health supplement. Any possible benefits come only from light-to-moderate intake, and they are easily outweighed by the well-known harms of alcohol. The safest amount of alcohol for health is none, and beer should be avoided in pregnancy and breastfeeding.
