Why this applies to Saquinavir
Saquinavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Bitter Melon interacts with.
Bitter melon might inhibit the p-glycoprotein (P-gp) intestinal pump and increase intracellular levels of P-gp substrates. In vitro research in intestinal cells shows that 1-monopalmitin, a constituent of bitter melon, increases levels of daunomycin, a P-gp substrate. Additionally, drinking bitter melon juice has been associated with a case of acute pancreatitis in a patient who had been taking pazopanib, a P-gp substrate, for 8 years. Researchers theorize that inhibition of P-gp led to increased levels of pazopanib, resulting in pazopanib-induced pancreatitis.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Saquinavir: your body uses a kind of cellular pump called P-glycoprotein (P-gp) to push this drug back out of your gut cells. Lab studies suggest a compound in bitter melon may slow that pump down. If the pump works less, a bit more saquinavir could get absorbed and stick around, which in theory could nudge up its levels and the chance of side effects like nausea, diarrhea, belly pain, headache, or changes in blood sugar and fats.
I want to be honest about the evidence though. This is mostly test-tube and animal data, not proven in people taking saquinavir, so it's a theoretical concern rather than a sure thing. Casual food amounts are far less likely to matter than concentrated supplement doses. Since saquinavir is an HIV medicine where steady, correct levels really matter, please check with your pharmacist or doctor before adding bitter melon.

Saquinavir is used in combination with ritonavir (Norvir) and other medications to treat human immunodeficiency virus (HIV) infection. Saquinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although saquinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
