Prednisone falls under P-GLYCOPROTEIN SUBSTRATES — the group Bitter Melon interacts with.

Theoretically, bitter melon might increase levels of P-glycoprotein substrates.
Bitter melon might inhibit the p-glycoprotein (P-gp) intestinal pump and increase intracellular levels of P-gp substrates. In vitro research in intestinal cells shows that 1-monopalmitin, a constituent of bitter melon, increases levels of daunomycin, a P-gp substrate. Additionally, drinking bitter melon juice has been associated with a case of acute pancreatitis in a patient who had been taking pazopanib, a P-gp substrate, for 8 years. Researchers theorize that inhibition of P-gp led to increased levels of pazopanib, resulting in pazopanib-induced pancreatitis.

Here's the short version: bitter melon may block a tiny pump in your gut called P-glycoprotein, which helps move certain drugs back out of your body. Prednisone is one of the medicines that pump can handle, so in theory, slowing that pump could let a bit more prednisone stay in your system. Prednisone is an active drug (not a prodrug), so more of it means stronger steroid effects could show up.

If that happened, you might notice things like trouble sleeping, mood swings, a puffy face, higher blood sugar, or fluid retention. But honestly, this concern comes only from lab and animal studies, not real patient data with prednisone, so it's theoretical. Food amounts in cooking are unlikely to matter. If you're using concentrated bitter melon supplements, just give your pharmacist or doctor a heads-up.
