Why this applies to Hydrocortisone

Hydrocortisone falls under P-GLYCOPROTEIN SUBSTRATES — the group Bitter Melon interacts with.

Theoretically, bitter melon might increase levels of P-glycoprotein substrates.
Bitter melon might inhibit the p-glycoprotein (P-gp) intestinal pump and increase intracellular levels of P-gp substrates. In vitro research in intestinal cells shows that 1-monopalmitin, a constituent of bitter melon, increases levels of daunomycin, a P-gp substrate. Additionally, drinking bitter melon juice has been associated with a case of acute pancreatitis in a patient who had been taking pazopanib, a P-gp substrate, for 8 years. Researchers theorize that inhibition of P-gp led to increased levels of pazopanib, resulting in pazopanib-induced pancreatitis.

Here's the idea behind pairing bitter melon with hydrocortisone. Your gut has a little pump called P-glycoprotein that helps push certain drugs back out before they're fully absorbed. Lab and animal studies suggest a compound in bitter melon may slow that pump down. Since hydrocortisone can travel through that same pump, the theory is that more of it could be absorbed, nudging its levels up a bit.

If that happened, you might notice more of the usual steroid effects, like fluid retention, higher blood sugar, mood changes, or trouble sleeping. I want to be honest though: this is theoretical, based only on test-tube and animal data, not real patient studies with hydrocortisone. For most people it's likely minor. Bitter melon may also lower blood sugar on its own, which is worth mentioning to your pharmacist or doctor before you combine them.

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
