Why this applies to Loperamide

Loperamide falls under P-GLYCOPROTEIN SUBSTRATES — the group Bitter Melon interacts with.

Theoretically, bitter melon might increase levels of P-glycoprotein substrates.
Bitter melon might inhibit the p-glycoprotein (P-gp) intestinal pump and increase intracellular levels of P-gp substrates. In vitro research in intestinal cells shows that 1-monopalmitin, a constituent of bitter melon, increases levels of daunomycin, a P-gp substrate. Additionally, drinking bitter melon juice has been associated with a case of acute pancreatitis in a patient who had been taking pazopanib, a P-gp substrate, for 8 years. Researchers theorize that inhibition of P-gp led to increased levels of pazopanib, resulting in pazopanib-induced pancreatitis.

Here's the gist with Imodium (loperamide) and bitter melon. Loperamide gets pushed out of your gut and brain by a little cellular pump called P-glycoprotein. Bitter melon may slow that pump down, which in theory could let a bit more loperamide hang around in your body instead of being shuttled away.

If that happens, you might feel loperamide's effects more strongly, things like constipation, cramping, or feeling drowsy. The bigger worry with too much loperamide is its effect on the heart's rhythm, though that's mostly seen at high doses. To be fair, this interaction is theoretical and based only on lab and animal data, so for normal short-term use it's likely minor. Still, if you use bitter melon supplements regularly, mention it to your pharmacist or doctor.

Loperamide controls the symptoms of diarrhea, including traveler's diarrhea, in adults and children 2 years and older. It is available as tablets, capsules, oral solution, and oral suspension across many brand names.

Loperamide Hydrochloride and Loperamide HCl capsules are also approved for chronic diarrhea in adults associated with inflammatory bowel disease, and for reducing the volume of discharge from an ileostomy (a surgical opening in the abdomen that drains waste).
