Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
Why this applies to Acetaminophen, Orphenadrine
Acetaminophen, Orphenadrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Black Pepper interacts with.
In vitro research suggests that black pepper induces CYP1A2. This has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for this combination pill, which pairs acetaminophen (a pain reliever) with orphenadrine (a muscle relaxer). One ingredient in black pepper, called piperine, may speed up a liver enzyme known as CYP1A2 that helps process certain medicines. In theory, that could clear the drug a little faster and make it work slightly less well, so you might notice a bit less pain or muscle relief.
The honest truth is that this is very weak evidence. It comes only from lab studies, not from real people, and it's rated minor. The sprinkle of pepper on your food won't matter at all. Even concentrated piperine supplements are unlikely to cause a real problem here, but if you take them often, it's worth a quick chat with your pharmacist.

About Black Pepper
What is Black Pepper?
- Flavoring and culinary use
- Digestive complaints like bloating and indigestion
- Boosting absorption of other supplements
- Antioxidant and anti-inflammatory support
- Cold and congestion remedies
Black pepper is a common kitchen spice that is generally safe in the amounts used in food. Its extract, piperine, is mostly added to supplements to help the body absorb other ingredients (like curcumin from turmeric). Strong human evidence for black pepper treating specific health conditions is limited, so use supplement-strength products with care and check with your pharmacist.
