PHENYTOIN (Dilantin)
Clinical research shows that piperine, a constituent of black pepper, seems to increase absorption, slow elimination, and increase levels of phenytoin. Taking a single dose of black pepper 1 gram along with phenytoin seems to double the serum concentration of phenytoin. Consuming a soup with black pepper providing piperine 44 mg/200 mL of soup along with phenytoin also seems to increase phenytoin levels when compared with consuming the same soup without black pepper.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's what's worth knowing if you take phenytoin (Dilantin). The piperine in black pepper can slow down how your body clears phenytoin, which lets more of it build up in your blood. In one small study, just 1 gram of black pepper roughly doubled phenytoin levels, and even a soup made with peppery piperine nudged levels up too.
Phenytoin is a narrow-therapeutic-index drug, meaning the gap between a helpful dose and too much is small. Higher levels can bring on dizziness, unsteady walking, slurred speech, double vision, or drowsiness. The sprinkle of pepper on your dinner is unlikely to cause trouble, but concentrated piperine supplements (often paired with turmeric) are a different story. What Is Phenytoin Used For?
Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection
Phenytoin treats seizures. Oral products are used for generalized tonic-clonic (grand mal) seizures and for psychomotor (temporal lobe) or complex partial seizures. Most oral products are also used to prevent and treat seizures during or after neurosurgery.
Phenytoin Sodium Injection is used for generalized tonic-clonic status epilepticus (a prolonged, dangerous seizure emergency). It is also used for seizures during neurosurgery. The injection should be used only when taking phenytoin by mouth is not possible. Dilantin-125 and Phenytoin oral suspension are listed for tonic-clonic and psychomotor seizures.
