Treprostinil falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Blood Irradiation Therapy interacts with.

Preliminary clinical research suggests that blood irradiation therapy might reduce platelet aggregation. Case reports also suggest that blood irradiation therapy may be associated with an increased risk of nasal bleeding. Theoretically, blood irradiation therapy might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
Some of these drugs include aspirin, clopidogrel (Plavix), nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Voltaren, Cataflam, others), ibuprofen (Advil, Motrin, others), naproxen (Anaprox, Naprosyn, others), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, warfarin (Coumadin), and others.

Here's the thing to know about treprostinil (Orenitram): it's used for pulmonary arterial hypertension, and on its own it already has a mild blood-thinning, anti-platelet effect (it relaxes blood vessels and makes platelets less sticky). Blood irradiation therapy is thought to do something similar, possibly making platelets clump together less. Stack the two and you could, in theory, raise your chances of bleeding or easy bruising.

I'll be honest, this concern is theoretical, based on how these two work rather than solid studies, and the reported bleeding was mostly minor (like nosebleeds). Still, watch for things like nosebleeds that won't quit, unusual bruising, or bleeding gums. Don't change anything on your own. Just loop in your doctor or pharmacist before combining them.

About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
