Naltrexone Hydrochloride falls under HEPATOTOXIC DRUGS — the group Buchu interacts with.

Buchu contains pulegone, a known hepatotoxin. There is some concern that buchu may adversely affect the liver, especially when the leaf is used in large doses or the oil is ingested. Theoretically, concomitant use with hepatotoxic drugs might increase the risk of liver damage. Some of these drugs include acarbose (Precose, Prandase), amiodarone (Cordarone), atorvastatin (Lipitor), azathioprine (Imuran), carbamazepine (Tegretol), cerivastatin (Baycol), diclofenac (Voltaren), felbamate (Felbatol), fenofibrate (Tricor), fluvastatin (Lescol), gemfibrozil (Lopid), isoniazid, itraconazole, (Sporanox), ketoconazole (Nizoral), leflunomide (Arava), lovastatin (Mevacor), methotrexate (Rheumatrex), nevirapine (Viramune), niacin, nitrofurantoin (Macrodantin), pioglitazone (Actos), pravastatin (Pravachol), pyrazinamide, rifampin (Rifadin), ritonavir (Norvir), rosiglitazone (Avandia), simvastatin (Zocor), tacrine (Cognex), tamoxifen, terbinafine (Lamisil), valproic acid, and zileuton (Zyflo)

Here's the thing to know about pairing buchu with naltrexone (Revia): this isn't about buchu changing how much naltrexone gets into your system. Instead, it's about the liver. Naltrexone can be a little hard on the liver, especially at higher doses, which is why your doctor may check liver enzymes before and during treatment. Buchu leaf contains a compound called pulegone that, at least on paper, may also stress the liver.

Stacking two things that lean on the liver could, in theory, raise the risk of liver irritation. I'll be honest, this concern is theoretical and mostly a worry with concentrated buchu oil or large leaf doses, not a casual cup of tea. Still, watch for yellowing skin or eyes, dark urine, or belly pain, and check with your pharmacist or doctor before combining them.

Buchu is a fragrant South African shrub whose leaves are traditionally used for urinary and bladder complaints and as a diuretic. Naltrexone Hydrochloride tablets are used to treat alcohol dependence and to block the effects of opioids in people with opioid dependence. The extended-release injectable suspension — sold as Vivitrol and also available as a generic injectable — is used to treat alcohol dependence and to prevent relapse to opioid dependence after a person has completed opioid detoxification.

All formulations are meant to be part of a broader treatment plan that includes counseling and other support — naltrexone works best when it is not used alone.
