Theoretically, caffeic acid might increase the levels and clinical effects of OAT3 substrates.
In vitro, caffeic acid inhibits OAT3. This drug transport protein is involved in renal tubular uptake of some drugs from the blood and then elimination in the urine. Inhibition of this transporters decreases renal elimination and increases drug levels in the body. Although caffeic acid inhibits OAT3 in vitro, the clinical significance of this in humans is not known.

Here's the gist for Avycaz (ceftazidime and avibactam). Your kidneys clear this antibiotic out of your body, partly through a little transporter called OAT3 that pumps the drug into your urine. In lab dish studies, caffeic acid appears to block that transporter. In theory, if that pump slows down, more of the antibiotic could linger in your bloodstream a bit longer.

I want to be honest about how shaky this is. This comes only from test-tube research, not from people, so we truly don't know if it matters in real life. Everyday coffee or fruit almost certainly won't be an issue. If you're considering a concentrated caffeic acid supplement while on Avycaz (which is given in the hospital anyway), just mention it to your care team first.

About Ceftazidime and Avibactam Injection

What Is Ceftazidime and Avibactam Injection Used For?

Brand name: Avycaz

Avycaz (ceftazidime and avibactam) is used to treat three types of serious bacterial infections in adults and children at least 31 weeks gestational age: complicated intra-abdominal infections (cIAI), complicated urinary tract infections (cUTI) including pyelonephritis (a kidney infection), and hospital-acquired bacterial pneumonia or ventilator-associated bacterial pneumonia (HABP/VABP). All three of these infections must be caused by specific susceptible gram-negative bacteria for Avycaz to be effective.

When treating complicated intra-abdominal infections, Avycaz must be used together with metronidazole, another antibiotic. Avycaz is only meant for infections proven or strongly suspected to be caused by bacteria that it can actually fight — using it otherwise can lead to antibiotic resistance.
