Acetaminophen, Caffeine, Dihydrocodeine falls under CNS DEPRESSANTS — the group Calamus interacts with.
Animal research shows that calamus is a CNS depressant and increases gamma-aminobutyric acid levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The thing to know here is that your pain medicine contains dihydrocodeine, an opioid that already slows down the brain and nervous system. Calamus, based on animal studies, seems to act as a mild sedative by raising a calming brain chemical called GABA. Stack the two together and you may feel more of the drowsy, foggy side: extra sleepiness, dizziness, slowed breathing, or trouble concentrating.
I'll be honest, this concern is theoretical and comes from lab and animal research, not solid human studies. Even so, calamus carries its own safety worries (the FDA banned it from food), so I'd steer clear of it altogether. Please check with your pharmacist or doctor before combining anything new with this medication.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that calamus extract inhibits CYP2D6 enzyme.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle worth knowing about this combo pill. The dihydrocodeine part is a prodrug, meaning your body has to switch it on using a liver enzyme called CYP2D6 to get the real pain relief. Lab studies suggest calamus may block that very enzyme. So instead of building up the drug, blocking CYP2D6 could leave you with less active pain relief, which might mean your pain isn't as well controlled.
I'll be honest, this is only theoretical and comes from test-tube research, not real people, so I wouldn't lose sleep over it. That said, calamus has its own safety concerns and is best avoided anyway. If you take this medication, please run it by your pharmacist or doctor first.

In vitro research shows that calamus extract inhibits CYP3A4 enzyme.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for this combo pill (Panlor, DHC Plus). Lab studies suggest calamus may slow down an enzyme in your body called CYP3A4. Since your medicine partly leans on that enzyme to be processed, blocking it could let some of the drug build up a bit more than usual. With dihydrocodeine especially, that might mean stronger sedation, more drowsiness, dizziness, constipation, or slowed breathing. The caffeine could also linger longer, leaving you jittery.
That said, this is only a theoretical concern from test-tube research, not proven in people, so don't panic. Honestly, calamus is best avoided anyway because of its own safety worries (it's banned from foods in the U.S.). If you're considering it, please run it by your pharmacist or doctor first.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.
