Animal research shows that calamus is a CNS depressant and increases gamma-aminobutyric acid levels.

Here's the simple version: Apadaz (benzhydrocodone with acetaminophen) already slows down your brain and nervous system, since the opioid part calms how you feel pain and can leave you drowsy. Calamus appears, mostly from animal studies, to act as a mild brain relaxant too, possibly by nudging up a calming chemical called GABA. Put them together and those calming effects could stack up.
What that might look like for you is extra sleepiness, dizziness, foggy thinking, or in a worst case, slower, shallower breathing. I'll be honest, this concern is theoretical and based on lab and animal work, not solid human studies. Calamus also has its own safety baggage, so before combining them, please check with your pharmacist or doctor.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that calamus extract inhibits CYP2D6 enzyme.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with Apadaz: the benzhydrocodone part is a prodrug. Your body has to use an enzyme called CYP2D6 to convert it into hydrocodone, which is what actually relieves your pain. Lab studies suggest calamus may block that same enzyme. If it does, less of the drug would turn into its active form, so you could get weaker pain relief than expected.
I'll be honest, this is only theoretical and comes from test-tube research, not real people, so we can't say how much it matters in practice. The bigger concern is calamus itself, which the FDA banned from foods over toxicity worries. I'd skip the supplement and check with your pharmacist or doctor before combining the two.

Here's the thing about Apadaz: the benzhydrocodone part is actually a prodrug. Your body has to convert it into hydrocodone, the form that actually relieves pain. Lab research hints that calamus may slow down an enzyme called CYP3A4, which is one of the pathways that helps clear hydrocodone once it's formed. If that pathway is blocked, hydrocodone could linger longer than expected.
In plain terms, that might mean stronger opioid effects: more drowsiness, dizziness, constipation, or slowed breathing in a worst-case scenario. I want to be honest though, this is only based on test-tube data, not real people, so it's theoretical. Calamus also carries its own safety concerns and is best avoided. Please check with your pharmacist or doctor before combining them.

Here's the honest picture for Apadaz (benzhydrocodone with acetaminophen). The interaction on file is grouped under anticholinergic drugs, but this pain medicine isn't really one of those. The reason it lands in that bucket is its calming, sedating side of things. Calamus has shown, only in lab dishes, that it can block an enzyme called acetylcholinesterase. That's a long way from proving anything happens in real people.
So I'd call this mostly theoretical. The bigger, well-known concern is simpler: both calamus and an opioid like this can make you drowsy, foggy, or lightheaded, so stacking them could deepen that. Calamus also has its own safety worries (it contains beta-asarone, which the FDA banned from foods). Please run this combination by your pharmacist or doctor before mixing them.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.

What Is Benzhydrocodone and Acetaminophen Used For?
Brand name: Apadaz
Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.
Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
