Caffeine (OTC drug) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Calamus interacts with.

Theoretically, taking calamus with drugs metabolized by CYP3A4 might increase drug levels and potentially increase the risk of adverse effects.

Here's the gist: lab studies suggest calamus can slow down a liver enzyme called CYP3A4. If a drug leans on that enzyme to be cleared from your body, it can hang around longer and hit harder. With caffeine, that could mean a bigger or longer-lasting buzz, things like jitteriness, a racing or pounding heart, trouble sleeping, or feeling anxious.

That said, I want to be honest about how thin this evidence is. It comes from test-tube research, not real people, and caffeine is actually broken down mostly by a different enzyme (CYP1A2), so the real-world effect here is likely small. The bigger concern with calamus is its own safety. If you take both, run it by your pharmacist or doctor first.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.
