Siltuximab falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Calamus interacts with.

Theoretically, taking calamus with drugs metabolized by CYP3A4 might increase drug levels and potentially increase the risk of adverse effects.
In vitro research shows that calamus extract inhibits CYP3A4 enzyme.

Here's the honest picture with Siltuximab (Sylvant) and calamus. The interaction on file is built around drugs that the liver enzyme CYP3A4 breaks down, and calamus may block that enzyme in lab tests. But Siltuximab is a monoclonal antibody, a large protein given by IV infusion. Antibodies like this aren't processed by CYP3A4 the way pills are. They're broken down into amino acids by the body, so this particular enzyme concern doesn't really apply to it.

This warning is also theoretical, coming only from test-tube and animal data, not from people. The bigger reason I'd steer you away from calamus is its own safety record (it contains beta-asarone, which the FDA banned from food). Please run any supplement past your oncology team before using it.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.

What Is Siltuximab Injection Used For?

Brand name: Sylvant

Sylvant (siltuximab) injection is approved to treat multicentric Castleman's disease (MCD) — a rare disorder involving abnormal lymph node growth driven by an overactive immune signal called IL-6. It is specifically approved for patients who test negative for both HIV and HHV-8 (human herpesvirus-8).

Sylvant has not been studied in patients with MCD who are HIV-positive or HHV-8-positive, because it does not block the form of IL-6 produced by those viruses.
