Theoretically, taking calamus with drugs metabolized by CYP2D6 might increase drug levels and potentially increase the risk of adverse effects.

Here's the gist with trazodone and calamus. Trazodone isn't a prodrug, so it works as-is. Your body partly clears it using a liver enzyme called CYP2D6. In lab (test tube) studies, calamus appears to slow that enzyme down. If that held true in people, trazodone could build up a bit higher than usual, which might mean stronger or longer-lasting side effects like drowsiness, dizziness, dry mouth, or low blood pressure when you stand up.

I want to be honest though: this is theoretical, based only on lab data, not real patients. On top of that, calamus carries its own safety worries (the FDA banned it from foods), so I'd lean away from it regardless. Please check with your pharmacist or doctor before combining them.

Here's the gist for trazodone: your liver leans on an enzyme called CYP3A4 to clear it out of your body. In lab (test tube) studies, calamus appears to slow that same enzyme down. If that happened in real people, trazodone could build up and hit harder than expected, meaning more drowsiness, dizziness, low blood pressure, and grogginess. Trazodone is already known for making people sleepy, so piling on isn't ideal.

That said, this is only theoretical right now, based on cell research, not human studies. So I wouldn't panic. Calamus also carries its own safety baggage (the FDA banned it from food), so it's worth a chat with your pharmacist or doctor before combining the two.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.
