Troglitazone falls under ANTIDIABETES DRUGS — the group Capers interacts with.

Some preliminary clinical research shows that taking caper fruit extract 1200 mg daily for 2 months can lower fasting blood glucose and glycated hemoglobin (HbA1c) in patients with type 2 diabetes who are already taking antidiabetes drugs. However, other research does not support this effect. Until more is known, monitor blood glucose levels closely. Medication dose adjustments may be necessary.
Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the gist with troglitazone (Rezulin), an older diabetes pill that helps your body use insulin better. The concern with concentrated caper fruit extract isn't about how your liver breaks the drug down. It's simpler than that: capers may have a mild blood-sugar-lowering effect of their own. So if you take a real supplement dose on top of your diabetes medicine, the two together could push your glucose lower than expected.

That can mean signs of low blood sugar, like shakiness, sweating, dizziness, or feeling foggy. The evidence here is fairly weak and the studies don't fully agree, so this isn't a reason to panic. Eating capers as food on a salad won't matter. If you're considering an extract, check your numbers more often and loop in your pharmacist or doctor first.
