Why this applies to Tasimelteon

Tasimelteon falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cat's Claw interacts with.

Theoretically, cat's claw might increase or decrease the levels and effects of drugs metabolized by CYP3A4.
Cat's claw may affect the clearance of drugs metabolized by CYP3A4. In vitro research shows that cat's claw can inhibit CYP3A4 enzymes. In one case report, a patient taking cat's claw (at an unspecified dose) experienced increased serum levels of atazanavir, ritonavir, and saquinavir, all of which are CYP3A4 substrates. Levels returned to normal 15 days after discontinuation of the cat's claw supplement, suggesting inhibition of CYP3A4 by cat's claw. In contrast, animal research suggests that rhynchophylline, an alkaloid contained in cat's claw, induces CYP3A expression and accelerates the metabolism of nirmatrelvir, the active component in the nirmatrelvir/ritonavir combination product.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to clear tasimelteon (Hetlioz). Cat's claw can tug on that same enzyme, so in theory it could nudge your tasimelteon levels in either direction. If it slows the enzyme, more drug hangs around and you might feel groggier, more drowsy the next day, or have headaches. If it speeds the enzyme up instead, the medicine could be weakened and your sleep-wake schedule may drift back out of sync.

I want to be honest about the strength here: this is based on lab and animal studies, not solid human data with tasimelteon, so it may never cause a real problem. Tasimelteon isn't a prodrug, so the drug itself is what's affected. Still, since timing matters so much for this medicine, please check with your pharmacist or doctor before adding cat's claw.

What Is Tasimelteon Used For?

Brand names: Hetlioz, Hetlioz Lq

Tasimelteon capsules are approved to treat Non-24-Hour Sleep-Wake Disorder (Non-24) in adults. Non-24 is a condition where the body's internal clock does not align with the normal 24-hour day, causing sleep and wake times to shift continuously.

Hetlioz capsules are also approved to treat nighttime sleep disturbances in Smith-Magenis Syndrome (SMS) in patients 16 years of age and older. Hetlioz LQ oral suspension is approved specifically for nighttime sleep disturbances in SMS in pediatric patients ages 3 to 15 years.
