Here's the simple version: chaparral has been tied to serious liver damage, and that's the real concern when pairing it with carfilzomib (Kyprolis). Carfilzomib isn't a prodrug, so this isn't about one speeding up or slowing down the other. Instead, both can stress the liver, and stacking them may add up. Carfilzomib already carries a risk of liver problems, including elevated liver enzymes and, rarely, more serious liver injury.

To be fair, this warning is mostly theoretical and based on weak, anecdotal reports, so I'm not saying disaster is coming. But honestly, with chaparral's track record and the FDA warning against it, I'd steer clear of it altogether. Please loop in your oncology team or pharmacist before adding any supplement during cancer treatment.

What Is Carfilzomib Injection Used For?

Brand name: Kyprolis

Kyprolis (carfilzomib) injection is used to treat adults with relapsed or refractory multiple myeloma — a cancer of plasma cells in the bone marrow — whose disease has come back or stopped responding to earlier treatment. It is approved for patients who have received one or more prior lines of therapy.

Kyprolis may be given alone or in combination with other medications, including lenalidomide and dexamethasone, dexamethasone alone, daratumumab and dexamethasone, daratumumab and hyaluronidase-fihj and dexamethasone, or isatuximab and dexamethasone. The specific combination depends on how many prior treatments a patient has received and what their care team recommends.
