Ipratropium falls under ANTICHOLINERGIC DRUGS — the group Clubmoss interacts with.

Evidence from in vitro research suggests that clubmoss extract can inhibit acetylcholinesterase activity. Theoretically, concurrent use of anticholinergic drugs and clubmoss might decrease the effectiveness of club moss or the anticholinergic agent. Some anticholinergic drugs include atropine, benztropine (Cogentin), biperiden (Akineton), procyclidine (Kemadrin), and trihexyphenidyl (Artane).

Here's the gist with ipratropium (Atrovent), the inhaler you use to open up your airways. It works as an anticholinergic, meaning it blocks acetylcholine to relax breathing muscles. Clubmoss appears to do the opposite in lab studies, raising acetylcholine by blocking the enzyme that breaks it down. In theory, the two could partly cancel each other out, so your ipratropium might not control wheezing or shortness of breath as well, and clubmoss may lose its effect too.

That said, this is based only on test-tube research, not real people, so it's far from proven. Since ipratropium is inhaled and acts mostly in the lungs, a big overlap is unlikely. Still, given clubmoss can be toxic, please check with your pharmacist or doctor before combining them.
