Chlorpromazine falls under PHENOTHIAZINES — the group Cocoa interacts with.

Theoretically, phenothiazines might increase the levels and adverse effects of caffeine.
Cocoa contains caffeine. Phenothiazines can reduce the metabolism of caffeine by inhibiting cytochrome P450 1A2 (CYP1A2).

Here's the interesting twist with this pairing: it's not that cocoa changes how your chlorpromazine works. It actually goes the other way. Chlorpromazine can slow down an enzyme in your liver (called CYP1A2) that normally breaks down caffeine, and cocoa naturally contains a little caffeine. So in theory, the caffeine could hang around longer and feel a bit stronger, meaning things like jitteriness, a faster heartbeat, or trouble sleeping.

I want to be straight with you though: this is rated minor and is based only on lab-type evidence, not real patient cases. The amount of caffeine in a cup of cocoa or some chocolate is quite small, so most people won't notice anything. If you use concentrated cocoa supplements or feel wired, just mention it to your pharmacist or doctor.

What Is Chlorpromazine Used For?

Brand names: Thorazine, Thorazine Spansules

Chlorpromazine is used to manage psychotic disorders and treat schizophrenia. It also controls nausea and vomiting, relieves restlessness and apprehension before surgery, and controls the manic phase of manic-depressive illness. Other uses include relief of intractable hiccups, treatment of acute intermittent porphyria, and use as an adjunct in tetanus treatment.

In children ages 1 to 12, it is used for severe behavioral problems marked by explosive or combative behavior, and for short-term treatment of hyperactive children with conduct disorders. The oral forms are also approved for the management of manifestations of psychotic disorders broadly, in addition to the uses shared with the injection form.
