Why this applies to Perphenazine
Perphenazine falls under PHENOTHIAZINES — the group Cocoa interacts with.
Cocoa contains caffeine. Phenothiazines can reduce the metabolism of caffeine by inhibiting cytochrome P450 1A2 (CYP1A2).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's something a little unusual about this pairing. The concern isn't that cocoa changes how your perphenazine (Trilafon) works. It's actually the other way around. Perphenazine can slow down a liver enzyme called CYP1A2, which is the same enzyme your body uses to clear caffeine. Since cocoa contains a small amount of caffeine, that caffeine could hang around a bit longer than usual.
In practice, this might mean you feel slightly more jittery, have a faster heartbeat, or have trouble sleeping if you take in a lot of caffeine. But honestly, the evidence here is weak (based mostly on lab studies), and the caffeine in a normal cup of hot cocoa or a chocolate treat is small. Concentrated cocoa supplements are a different story. If you use those, or you're caffeine-sensitive, just mention it to your pharmacist.

Perphenazine tablets are approved to treat schizophrenia in adults. They are also used to control severe nausea and vomiting in adults when other measures are not enough.
Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
