Here is the interaction we found between these two, with the evidence behind it.
TIAGABINE (Gabitril)
Cocoa contains caffeine. Animal research suggests that chronic caffeine administration can increase the serum concentrations of tiagabine. However, concomitant use does not seem to reduce the antiepileptic effects of tiagabine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with cocoa and your seizure medicine tiagabine. Cocoa naturally contains a little caffeine, and in animal studies, taking caffeine regularly seemed to nudge tiagabine levels in the blood a bit higher. If that happened in people, it could mean slightly stronger effects from the drug, like more drowsiness, dizziness, trouble concentrating, or feeling shaky. The good news is the same research suggested it didn't make tiagabine work worse for seizures.
I want to be honest with you: this is a theoretical, low-level concern based only on lab and animal data. The small amount of cocoa in a cup of hot chocolate or a piece of chocolate is very unlikely to cause problems. Concentrated cocoa supplements are a different story, so check with your pharmacist before starting one.

From our pharmacist’s overview on the Tiagabine page
“Anyone taking tiagabine (Gabitril) for seizures should treat its supplement interactions with real vigilance. The drug is cleared by the liver enzyme CYP3A4, so grapefruit can push its blood level up while St.”

About Cocoa
What is Cocoa?
- Heart and blood vessel health
- High blood pressure
- High cholesterol
- Mental focus and mood
- Skin moisturizing (cocoa butter)
Cocoa is rich in plant compounds called flavanols that may modestly support blood vessel function and blood pressure, but most chocolate products are high in sugar, fat, and calories, which can offset any benefit. Enjoy cocoa as part of a balanced diet, and talk to your pharmacist before using concentrated cocoa supplements, especially if you have heart or blood pressure conditions.

Tiagabine Is Linked to Lower Levels of One Nutrient
Natural Medicines records Tiagabine as a medicine that can lower these. This is not an interaction with Cocoa — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
