Anyone taking tiagabine (Gabitril) for seizures should treat its supplement interactions with real vigilance. The drug is cleared by the liver enzyme CYP3A4, so grapefruit can push its blood level up while St. John's wort can pull it down, and with a seizure medicine that swing cuts both ways, toward heavier side effects or weaker seizure control. Alcohol and dozens of sedating supplements also pile onto the drowsiness tiagabine already causes, and that combination accounts for several of the most serious warnings.

A smaller cluster, charcoal and bulky fibers among them, can block absorption if swallowed near a dose, so space those well apart. A few others may lower the seizure threshold on their own.
