Cariprazine Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Coleus interacts with.

Theoretically, taking coleus might decrease serum levels of drugs metabolized by CYP3A4.
In vitro research shows that coleus can activate the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of coleus and other drugs affected by these enzymes.

Here's the gist for Vraylar (cariprazine). Your liver uses an enzyme called CYP3A4 to process this medication. In lab and animal studies, Coleus (from its forskolin content) appears to speed up that enzyme, which in theory could clear cariprazine out of your system a little faster. If that happened, you might get less benefit, meaning your mood, manic, or schizophrenia symptoms could be less well controlled.

I want to be honest about the strength of this: it's based only on test-tube and animal data, not real patient studies, so the actual effect in people is unknown. Cariprazine also has very long-acting active byproducts, which may soften any change. Still, before adding Coleus, please check with your pharmacist or doctor so we can keep an eye on how you're feeling.

What Is Cariprazine Used For?

Brand name: Vraylar

Vraylar (cariprazine) is approved to treat schizophrenia in adults and in adolescents aged 13 and older. It is also used for the acute treatment of manic or mixed episodes associated with bipolar I disorder in adults and in children aged 10 and older.

In adults, Vraylar is additionally approved to treat depressive episodes associated with bipolar I disorder (bipolar depression) and as an add-on (adjunctive) therapy alongside antidepressants for major depressive disorder (MDD).
