Why this applies to Dronabinol

Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Coleus interacts with.

Theoretically, taking coleus may affect drugs metabolized by CYP2C9 and increase the risk of adverse effects or reduce the effectiveness.
Research on the effect of coleus on CYP2C9 is conflicting. Some animal research shows that coleus extract can induce CYP2C9, while in vitro research shows that coleus can inhibit CYP2C9. Until more is known, advise patients that taking coleus might increase or decrease levels of drugs metabolized by CYP2C9.

Here's the situation with Coleus and your dronabinol. Your body partly uses a liver enzyme called CYP2C9 to clear dronabinol out of your system. Coleus (and its active compound forskolin) may bump into that same enzyme. The tricky part is that the research points in two directions: some lab work suggests Coleus could slow this enzyme down, while some animal studies suggest it could speed it up.

So in theory, Coleus could either let dronabinol build up (more drowsiness, dizziness, confusion, or feeling 'high') or clear it faster (less help with nausea or appetite). Honestly, this is based only on lab and animal data, so it's a maybe, not a sure thing. Still, I'd run it past your doctor or pharmacist before combining them, just to be safe.

Dronabinol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, taking coleus might decrease serum levels of drugs metabolized by CYP3A4.
In vitro research shows that coleus can activate the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of coleus and other drugs affected by these enzymes.

Here's the gist: your body clears dronabinol (the active ingredient in Marinol and Syndros) partly through a liver enzyme called CYP3A4. Lab studies suggest Coleus may nudge that enzyme to work faster. If that happens in real life, your system could break down dronabinol a bit quicker, which might mean less relief from nausea, vomiting, or poor appetite, the very things you're taking it for.

I want to be honest about the strength of this, though. This concern comes only from test-tube and cell research, not from people, so we really don't know if it matters in everyday use. Dronabinol isn't a prodrug, so the worry is weaker effect, not extra side effects. Before mixing the two, check with your pharmacist or doctor so they can keep an eye on how well your dronabinol is working.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
