Why this applies to Losartan Potassium

Losartan Potassium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Delta-9-tetrahydrocannabinol (thc) interacts with.

Theoretically, THC might increase the levels and adverse effects of CYP2C9 substrates.
In vitro research shows that THC moderately inhibits the CYP2C9-mediated 7-hydroxylation of S-warfarin in a concentration-dependent manner. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP2C9 metabolism of tolbutamide; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Theoretically, THC may inhibit the metabolism of other CYP2C9 substrates. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP2C9.

Here's a wrinkle worth knowing about losartan: it actually needs your liver's CYP2C9 enzyme to turn it into its more active form. So unlike most drugs where blocking that enzyme makes them build up, losartan works a little backward. In theory, if THC slowed down CYP2C9, you might get slightly less of losartan's blood-pressure-lowering punch, not more.

The good news is that this concern comes only from lab dish studies, and a small human study actually found oral THC did not block CYP2C9 in real people. So I wouldn't lose sleep over it. Still, if you use cannabis regularly and notice your blood pressure creeping up, mention it to your pharmacist or doctor so they can check.

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Here's the simple version for losartan (Cozaar). Losartan gets switched on in your liver by an enzyme called CYP3A4, which helps turn it into its more active form. THC has shown, mostly in lab dishes, that it might slow that same enzyme down a little. Because losartan partly depends on this enzyme to become fully active, the theory cuts both ways and the real-world effect is honestly murky.

The good news: this is rated minor, unlikely, and rests only on test-tube data. In fact, one human study found oral THC did not block CYP3A4 at all. So a meaningful change in your blood pressure control or extra dizziness from losartan is improbable. Still, since both can lower blood pressure and THC can make you lightheaded, just give your pharmacist or doctor a heads-up.

What Is Losartan Used For?

Brand name: Cozaar

Losartan treats high blood pressure (hypertension) in adults and children 6 years and older. Keeping blood pressure under control lowers the risk of serious events like stroke and heart attack.

It is also used to reduce the risk of stroke in people who have high blood pressure and left ventricular hypertrophy (an enlarged heart muscle). Note: evidence suggests this stroke-risk benefit may not apply to Black patients. Additionally, losartan treats diabetic nephropathy — kidney damage caused by type 2 diabetes — in people who also have high blood pressure.
