Here's the thing worth knowing if you're using THC and taking Ibrance (palbociclib). Palbociclib is a mild blocker of an enzyme called CYP3A4, and THC happens to be cleared by that same enzyme. So in theory, palbociclib could slow how fast your body breaks down THC, letting THC build up a bit more than usual. That could mean a stronger or longer 'high,' more drowsiness, dizziness, or foggy thinking.

I want to be honest that this is a theoretical concern based on how these compounds are processed, not something proven in real patients. Palbociclib itself isn't changed by THC here. Still, since you're managing breast cancer treatment, it's smart to mention any cannabis use to your oncologist or pharmacist so they can keep an eye on how you're feeling.

Palbociclib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Here's the gist for Ibrance (palbociclib): your body relies on a liver enzyme called CYP3A4 to break it down. Lab studies suggest THC might slow that enzyme a little, which in theory could let palbociclib build up slightly. If that happened, you could see more of its usual side effects, things like low white blood cell counts (which raises infection risk), fatigue, mouth sores, or nausea.

That said, I wouldn't lose sleep over this one. The concern comes only from test-tube work, and a human study using oral THC found it did not block CYP3A4. The interaction is rated minor and unlikely. Still, since palbociclib is a serious cancer medicine, please tell your oncologist and pharmacist if you use THC so they can keep an eye on your bloodwork.

About Palbociclib

What Is Palbociclib Used For?

Brand name: Ibrance

Palbociclib (brand name Ibrance) is used to treat adults with hormone receptor (HR)-positive, HER2-negative advanced or metastatic breast cancer. It is always given in combination with other medicines — either an aromatase inhibitor as a first hormonal treatment, or with fulvestrant after the cancer has progressed on prior hormonal therapy. It is also used with inavolisib and fulvestrant for a specific subtype (PIK3CA-mutated, endocrine-resistant) of this cancer confirmed by an FDA-authorized test.

Ibrance is also approved in combination with trastuzumab (with or without pertuzumab) and endocrine therapy for maintenance treatment of adults with HR-positive, HER2-positive locally advanced or metastatic breast cancer, following initial induction treatment.
