Neostigmine Methylsulfate falls under CHOLINERGIC DRUGS — the group Dimethylsulfoxide (dmso) interacts with.

Theoretically, DMSO might increase the risk of cholinergic adverse effects when taken with cholinergic drugs.

Here's the simple version. Neostigmine works by blocking an enzyme called acetylcholinesterase, which lets your body's natural acetylcholine build up. DMSO is thought to do something similar in lab and animal studies, so in theory using both could push that effect a little further than intended.

If that happened, you might see more of neostigmine's usual cholinergic side effects, things like extra saliva or drooling, watery eyes, an upset or crampy stomach, nausea, a slower heart rate, or muscle twitching. I want to be honest though: this is a theoretical concern based on test-tube and animal data, not real patient reports. Neostigmine is usually given in a monitored setting anyway. Still, mention any DMSO use to your doctor or pharmacist so they can keep an eye on you.
