pioglitazone, metformin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Eucalyptus interacts with.

Theoretically, eucalyptus might increase the levels of CYP3A4 substrates.
In vitro research suggests that eucalyptus oil might inhibit CYP3A4, although this has not been reported in humans.

Here's the quick picture for your ActoPlus Met (pioglitazone plus metformin). Lab-dish studies hint that eucalyptus oil might slow down a liver enzyme called CYP3A4. Pioglitazone leans on that enzyme to be cleared from your body, so in theory, blocking it could let pioglitazone build up a bit. If that happened, you might notice more of its usual effects, like fluid retention, swelling in the ankles, weight gain, or low blood sugar (shaky, sweaty, dizzy feelings). The metformin half isn't really handled by this enzyme, so it's not the concern here.

Honestly, this is only a test-tube theory and has never been seen in real people, especially from sniffing a vapor rub or sucking a lozenge. Still, before swallowing concentrated eucalyptus products, check with your pharmacist or doctor.

Here's the gist: ActoPlus Met combines two diabetes medicines (pioglitazone and metformin) that already work to bring your blood sugar down. Some animal studies hint that eucalyptus leaf may also have a mild blood-sugar-lowering effect of its own. If that holds true in people, stacking the two could nudge your levels a little lower than expected, raising the chance of a low (shakiness, sweating, dizziness, confusion).

I want to keep this in perspective, though. This is only a theoretical concern based on lab and animal work, not solid human data, and the strong-smelling oil shouldn't be swallowed anyway. Inhaling eucalyptus for a stuffy nose is unlikely to move your sugar at all. If you're using eucalyptus leaf products by mouth, just check your readings and mention it to your pharmacist or doctor first.
