Cangrelor falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group European Barberry interacts with.

Theoretically, European barberry may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal evidence suggest that berberine, a constituent of European barberry, might inhibit platelet aggregation. Theoretically, European barberry might have a similar effect.

Here's the practical picture. Cangrelor (Kengreal) is a powerful IV antiplatelet medicine given in the hospital, usually during procedures like a cardiac stent placement, to keep platelets from clumping and forming clots. It is not a prodrug, so it works directly the moment it's infused. European barberry contains berberine, which in lab and animal studies seems to gently discourage platelets from sticking together too. Stack a blood-thinning herb on top of an already strong antiplatelet drug, and on paper you could see more bruising or bleeding.

That said, this is a theoretical concern based on test-tube and animal data, not human cases. Realistically, cangrelor is given under close medical supervision, so just be honest with your care team about any barberry or berberine supplements you take.
