Ibuprofen, Hydrocodone falls under CNS DEPRESSANTS — the group European Barberry interacts with.
Animal research suggests that berberine, a constituent of European barberry, might have sedative effects. Theoretically, European barberry might have a similar effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The combo medicine you're asking about pairs ibuprofen (an anti-inflammatory) with hydrocodone, an opioid that can make you drowsy. Hydrocodone already slows things down a bit in the brain. The thought here is that berberine, the main alkaloid in European barberry, showed mild sedative effects in animal studies. So in theory, stacking barberry on top of hydrocodone could add to that sleepy, foggy feeling.
This is a theoretical concern only, based on lab and animal work, not on real-world cases, so I don't want you to panic. Still, if you notice extra drowsiness, dizziness, or slowed thinking, that's worth paying attention to. Before adding a barberry or berberine supplement, please run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Ibuprofen, Hydrocodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo pill pairs hydrocodone (an opioid pain reliever) with ibuprofen (an anti-inflammatory). The piece that matters here is the hydrocodone side, since your liver uses an enzyme called CYP3A4 to help process it. Berberine, the main active compound in European barberry, has shown in very early lab research that it might slow that enzyme down. If it does, hydrocodone could hang around a bit longer and feel stronger than expected, which can mean more drowsiness, dizziness, nausea, constipation, or in larger amounts, slowed breathing.
Keep in mind this is purely theoretical and not proven in people, so I wouldn't lose sleep over it. Still, before adding a barberry or berberine supplement, run it past your pharmacist or doctor so they can weigh it for you.

ANTICOAGULANT/ANTIPLATELET DRUGS

In vitro and animal evidence suggest that berberine, a constituent of European barberry, might inhibit platelet aggregation. Theoretically, European barberry might have a similar effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with this combo (Ibucodone is ibuprofen plus hydrocodone). The ibuprofen half is an NSAID, and NSAIDs already have a mild blood-thinning effect because they make platelets a bit less sticky. Barberry contains berberine, which in lab and animal studies also seems to slow platelets down. So in theory, taking both could nudge your bleeding risk a little higher.
What that might look like in real life: easier bruising, nosebleeds, stomach upset, or dark or tarry stools that could signal a stomach bleed. I'll be honest, this concern is theoretical and not proven in people, so don't panic. Just loop in your pharmacist or doctor before adding a concentrated barberry or berberine supplement, especially if you take it daily.
