Imatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group European Barberry interacts with.

Theoretically, European barberry might increase the levels and clinical effects of CYP3A4 substrates.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.

Here's the situation with Gleevec (imatinib) and European barberry. Your liver relies on an enzyme called CYP3A4 to clear imatinib out of your system. Berberine, the main active compound in barberry, may slow that enzyme down a bit. If it does, imatinib could clear more slowly and build up in your blood, which can mean stronger side effects like nausea, fluid buildup and swelling (around the eyes or in the legs), muscle cramps, fatigue, or lower blood counts.

I want to be honest about the strength of this: it's theoretical, based on how berberine behaves in the lab, not on real patient cases. Still, imatinib is a serious cancer medicine, so I wouldn't combine the two on your own. Please check with your oncologist or pharmacist first.

European barberry is a shrub whose root, bark, and berries contain berberine, a bitter plant alkaloid that has been studied mostly in isolated form. Some early research on berberine is promising for blood sugar and cholesterol, but high-quality evidence for the whole barberry herb is limited. What Is Imatinib Used For?

Brand name: Gleevec

Imatinib treats a range of blood cancers and solid tumors. It is used for newly diagnosed and previously treated Philadelphia chromosome positive chronic myeloid leukemia (Ph+ CML) in adults and children, Philadelphia chromosome positive acute lymphoblastic leukemia (Ph+ ALL) in adults and children, myelodysplastic/myeloproliferative diseases (MDS/MPD) linked to certain gene changes, aggressive systemic mastocytosis (ASM), hypereosinophilic syndrome (HES) and/or chronic eosinophilic leukemia (CEL), dermatofibrosarcoma protuberans (DFSP, a type of skin tumor), and Kit-positive gastrointestinal stromal tumors (GIST) — including after surgical removal.

All of these uses are supported by the tablet form (Gleevec, Imatinib, Imatinib Mesylate) and the oral solution (Imkeldi). The same full set of indications applies across products, but every product-specific instruction should be followed as directed by your prescriber.
