Why this applies to Apremilast

Apremilast falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fennel interacts with.

Theoretically, fennel might increase levels of drugs metabolized by CYP3A4.

In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.

Here's the gist for Otezla (apremilast): your body clears it mainly through a liver enzyme called CYP3A4. In test-tube studies, fennel appears to slow that enzyme down a bit. If that happened in people, apremilast could linger longer in your system and its effects could feel stronger. With apremilast, that usually means more of the common side effects, things like nausea, diarrhea, headache, or stomach upset.

The important part: this is only theoretical lab data, and it has not been shown in real people. Everyday fennel in cooking or tea is very unlikely to cause trouble. Concentrated fennel supplements or the essential oil are where I'd be more cautious, so just run those by your pharmacist or doctor before starting them.

Fennel is a Mediterranean herb widely used as a food and spice, and traditionally taken for digestive complaints, colic, and menstrual cramps. Some small studies suggest possible benefit for certain stomach and menstrual symptoms, but the evidence is limited. It is generally well tolerated in food amounts, but concentrated supplements and the essential oil should be used with caution and after checking with your pharmacist or doctor.

What Is Apremilast Used For?

Brand name: Otezla

Apremilast treats several immune-related conditions. In adults, it is used for active psoriatic arthritis (joint inflammation linked to psoriasis), plaque psoriasis in people who are candidates for light therapy or systemic treatment, and oral ulcers associated with Behçet's Disease (a rare inflammatory disorder causing mouth sores).

In children 6 years of age and older, Otezla is approved for active psoriatic arthritis and moderate to severe plaque psoriasis in patients weighing at least 20 kg. Otezla is the only brand supported by the evidence on this page; it is taken as an oral tablet.
