Brexpiprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fennel interacts with.

Theoretically, fennel might increase levels of drugs metabolized by CYP3A4.

In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.

Here's the gist for Rexulti (brexpiprazole): your body relies on a liver enzyme called CYP3A4 to clear it out. Lab studies hint that fennel might slow that enzyme down a bit. If that happened in real life, brexpiprazole could build up slightly and you might notice more of its usual effects, like drowsiness, restlessness, dizziness, or feeling foggy.

The reassuring part is that this is only seen in test-tube research, never in actual people, so I'd put it in the theoretical bucket. Fennel as a food or spice almost certainly won't matter. Concentrated fennel supplements or the essential oil are where I'd be a little more careful. If you plan to use those regularly, just run it by your pharmacist or doctor first.

Fennel is a Mediterranean herb widely used as a food and spice, and traditionally taken for digestive complaints, colic, and menstrual cramps. Some small studies suggest possible benefit for certain stomach and menstrual symptoms, but the evidence is limited. About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
