Cabozantinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fennel interacts with.

Theoretically, fennel might increase levels of drugs metabolized by CYP3A4.

In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.

Here's the basic idea with fennel and cabozantinib. Your body relies on a liver enzyme called CYP3A4 to clear cabozantinib out of your system. Lab studies hint that fennel may slow that same enzyme down. If that happened in people, cabozantinib could build up a bit higher than expected, which might mean more of its typical side effects, things like diarrhea, mouth sores, high blood pressure, fatigue, hand-foot skin reactions, or nausea.

The important part: this is theoretical. It's only been seen in test-tube research, never in actual patients, and everyday cooking amounts of fennel are very unlikely to matter. Concentrated fennel supplements or the essential oil are a different story. Since cabozantinib is a serious cancer medicine, just run any fennel supplement past your oncologist or pharmacist first.

Fennel is a Mediterranean herb widely used as a food and spice, and traditionally taken for digestive complaints, colic, and menstrual cramps. Some small studies suggest possible benefit for certain stomach and menstrual symptoms, but the evidence is limited. It is generally well tolerated in food amounts, but concentrated supplements and the essential oil should be used with caution and after checking with your pharmacist or doctor.

About Cabozantinib

What Is Cabozantinib Used For?

Brand name: Cometriq

Cabozantinib treats several types of cancer. Cabometyx (tablet) is used for advanced renal cell carcinoma (kidney cancer) — either alone or combined with nivolumab as a first-line treatment — as well as hepatocellular carcinoma (liver cancer) in patients previously treated with sorafenib, differentiated thyroid cancer that has progressed after prior therapy, and pancreatic or extra-pancreatic neuroendocrine tumors (NETs) that have been previously treated. For thyroid cancer and NETs, Cabometyx is approved for adults and adolescents aged 12 and older.

Cometriq (capsule) is approved specifically for progressive, metastatic medullary thyroid cancer (a different type of thyroid cancer). Cometriq and Cabometyx are not interchangeable — do not substitute one for the other.
