Cabozantinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Acacia Rigidula interacts with.

Theoretically, Acacia rigidula products that are adulterated with beta-methylphenylethylamine (BMPEA) might increase the levels of CYP3A4 substrates.
In vitro research shows that BMPEA, an adulterant found in many Acacia rigidula products, inhibits CYP3A4 enzyme by 37%.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear cabozantinib out of your system. Some Acacia rigidula products have been found spiked with a hidden stimulant called BMPEA, and lab studies suggest BMPEA can slow that enzyme down by about a third. If that happened in a real person, cabozantinib could build up to higher levels than intended, which may mean more of its known side effects, things like diarrhea, mouth sores, high blood pressure, fatigue, and hand-foot skin reactions.

I want to be honest about the evidence here: this is theoretical, based on test-tube work, not on people. Still, cabozantinib is a serious cancer drug, so I wouldn't take chances. Please run any Acacia rigidula product past your oncology team or pharmacist first.

Acacia rigidula is a shrub from Texas and Mexico that is marketed in weight-loss and energy supplements. There is very little reliable human evidence that it works, and some products have been found to contain an unapproved stimulant drug (BMPEA), which raises real safety concerns. About Cabozantinib

What Is Cabozantinib Used For?

Brand name: Cometriq

Cabozantinib treats several types of cancer. Cabometyx (tablet) is used for advanced renal cell carcinoma (kidney cancer) — either alone or combined with nivolumab as a first-line treatment — as well as hepatocellular carcinoma (liver cancer) in patients previously treated with sorafenib, differentiated thyroid cancer that has progressed after prior therapy, and pancreatic or extra-pancreatic neuroendocrine tumors (NETs) that have been previously treated. For thyroid cancer and NETs, Cabometyx is approved for adults and adolescents aged 12 and older.

Cometriq (capsule) is approved specifically for progressive, metastatic medullary thyroid cancer (a different type of thyroid cancer). Cometriq and Cabometyx are not interchangeable — do not substitute one for the other.
