Why this applies to Macitentan, Tadalafil
Macitentan, Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fennel interacts with.
In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Opsynvi combines two medicines, macitentan and tadalafil, and both are cleared by a liver enzyme called CYP3A4. In lab dish studies, fennel appears to slow that enzyme down. If that same effect happened in people, these two drugs could hang around longer and build up, which might mean stronger effects and more side effects. With tadalafil you might notice things like headache, flushing, stuffy nose, or dizziness from low blood pressure. Macitentan can add to that light-headed feeling.
Here is the reassuring part: this is only a test-tube finding and has not been seen in real patients. Everyday amounts of fennel in cooking are very unlikely to matter. Concentrated fennel capsules or the essential oil are where I would pause. Just run those by your pharmacist or doctor first.
