Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fo-ti interacts with.
Animal research suggests that fo-ti might inhibit CYP2D6. Additionally, an in vitro study suggests that the degree of CYP2D6 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo pill: the hydrocodone piece relies on a liver enzyme called CYP2D6 to convert into its more active pain-relieving form. Lab and animal studies hint that Fo-ti might slow that enzyme down. If that held true in people, you could actually get less pain relief from hydrocodone, since less of it gets switched on. The other ingredients (acetaminophen, caffeine, chlorpheniramine, phenylephrine) aren't the main concern here.
I want to be honest, though: this is purely theoretical, based on test-tube and animal work, and it hasn't been seen in real patients. Raw Fo-ti seems to block the enzyme more than processed extracts do. If you take this medicine regularly, just mention the Fo-ti to your pharmacist or doctor so they can keep an eye on whether it's still controlling your pain.

In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.

The piece of this combination product that matters most here is the caffeine. Your body clears caffeine mainly through a liver enzyme called CYP1A2, and lab tests suggest Fo-ti might slow that enzyme down. If that happened, caffeine could linger longer than usual, so you might feel jittery, have a racing heart, feel anxious, or have trouble sleeping.
I want to be honest about how shaky this evidence is. It comes only from test-tube and animal studies, and the results were mixed (raw Fo-ti blocked the enzyme, while an alcohol extract sped it up). Nothing like this has been confirmed in people. The other ingredients aren't really driven by this enzyme. Still, since Fo-ti has its own liver-safety concerns, please run it by your pharmacist or doctor before combining them.

Here's the piece worth knowing about this combo product (sold as Hycomine Compound): it contains acetaminophen, which your liver has to process. Acetaminophen is already one of the most common causes of liver strain when taken in high doses or with other things that tax the liver. Fo-ti has been tied to liver injury in a number of case reports, so pairing the two could, in theory, add up to more stress on your liver.
I want to be honest about the evidence here: this is based on anecdotal reports, not strong studies, so it's a caution rather than a proven danger. Still, watch for signs like yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness. Please don't stop either one on your own. Just check in with your pharmacist or doctor first.

In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

This combo pill packs several ingredients, and the one that matters most here is hydrocodone, the narcotic cough and pain component. Hydrocodone is partly broken down by an enzyme in your liver called CYP3A4. Lab studies hint that Fo-ti might slow that enzyme down, which could let hydrocodone build up a bit more than usual. If that happened, you might feel more drowsy, dizzy, foggy, or constipated, and in larger amounts opioids can slow breathing.
Here's the honest part: this is only a theoretical concern from test-tube and animal research, and it has never actually been reported in people. Processed Fo-ti seems to have an even weaker effect. Still, since Fo-ti has been tied to liver injury and acetaminophen is also processed by the liver, it's worth a quick chat with your pharmacist or doctor before pairing them.
