Why this applies to Bedaquiline

Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Your body relies on a liver enzyme called CYP3A4 to break down bedaquiline. In test-tube studies, Fo-ti has shown the ability to slow that same enzyme down. If that happened in people, bedaquiline could clear more slowly and build up in your system, which might mean stronger effects and a higher chance of side effects.

With bedaquiline, the side effect I watch most closely is its impact on your heart rhythm (a QT prolongation that can cause dizziness or fainting), along with nausea, joint pain, and liver stress. Since Fo-ti itself has been linked to liver injury, that overlap is worth respecting. Still, this is theoretical lab-and-animal data, not something seen in real patients, and the results even conflict. Because bedaquiline is part of a serious TB regimen, please loop in your doctor or pharmacist before combining them.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
