Why this applies to Defibrotide Sodium

Defibrotide Sodium falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Fo-ti interacts with.

Fo-ti has been linked to cases of acute liver failure which can decrease clotting factor production and increase the effects of anticoagulants. In one case, a patient who had been stable on warfarin presented with acute hepatitis and an INR elevated to 14.98. The patient had been taking fo-ti for 90 days prior to admission. Discontinuation of warfarin and fo-ti lead to a decrease in the INR and full recovery. Theoretically, concomitant use of fo-ti with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients. Until more is known, monitor patients taking fo-ti and drugs that affect bleeding.
Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), dipyridamole (Persantine), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here is the thing to keep in mind with this combination. Defibrotide is a clot-protecting medicine given in the hospital to people who already have serious liver trouble after a stem-cell transplant. The worry with Fo-ti is twofold. First, it has been tied to cases of liver injury, and a hurting liver makes fewer clotting factors, which can tip you toward bleeding. Second, Fo-ti itself may mildly nudge up bleeding risk when paired with blood thinners.

Put together, that could mean a higher chance of bruising, nosebleeds, bleeding gums, or harder-to-stop bleeding in an already fragile situation. To be fair, the evidence here is thin and mostly anecdotal, not proven. Still, since defibrotide is reserved for very sick patients, please loop in your transplant team or pharmacist before using Fo-ti.

What Is Defibrotide Injection Used For?

Brand name: Defitelio

Defitelio (defibrotide injection) is used to treat hepatic veno-occlusive disease (VOD) — also called sinusoidal obstruction syndrome (SOS) — in adult and pediatric patients. This is a serious condition where small blood vessels in the liver become blocked, most often after a hematopoietic stem-cell transplant (HSCT).

Defitelio is specifically indicated when VOD is accompanied by kidney dysfunction or pulmonary (lung) dysfunction, making it a treatment for one of the most severe forms of this complication.
