Why this applies to Estradiol, Norgestimate

Estradiol, Norgestimate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase or decrease the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.

If you take Ortho-Prefest (estradiol plus norgestimate), here's the honest picture with Fo-ti. The estrogen part is partly handled by a liver enzyme called CYP1A2, and lab studies hint that Fo-ti might either slow that enzyme down or speed it up. These are neither drugs that get activated by the enzyme, so this isn't a prodrug situation. If Fo-ti slows the enzyme, estrogen could build up a bit (think breast tenderness, bloating, nausea, or spotting). If it speeds things up, the hormones might work less reliably.

The key thing: this is only test-tube and animal data, never confirmed in people, so I wouldn't lose sleep over it. Still, since Fo-ti has been tied to liver injury, please run it by your pharmacist or doctor before combining them.

Here's the gist with Ortho-Prefest, which combines estradiol (an estrogen) and norgestimate (a progestin) for menopausal hormone therapy. Lab studies suggest Fo-ti has some weak estrogen-like activity and may compete for the same estrogen receptors your medication is working through. In theory, large amounts of concentrated Fo-ti could nudge how well your hormone therapy does its job, possibly affecting symptom control like hot flashes, or contributing to things like spotting or breast tenderness.

I want to be honest about the strength of this: it comes only from test-tube research, not real people, so the everyday impact is uncertain. Occasional culinary use is unlikely to matter much more than high-dose supplements. Before adding Fo-ti, check with your pharmacist or doctor, especially since Fo-ti has its own liver-injury concerns.
