Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2D6.
Animal research suggests that fo-ti might inhibit CYP2D6. Additionally, an in vitro study suggests that the degree of CYP2D6 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this interaction has not been reported in humans.

Here's the wrinkle with this combo: hydrocodone is actually a prodrug for part of its pain-relieving punch. Your body uses an enzyme called CYP2D6 to convert some hydrocodone into hydromorphone, a stronger painkiller. Lab and animal studies hint that Fo-ti (especially the raw root) may slow that same enzyme. In theory, that could mean a bit less conversion and slightly weaker pain relief from the hydrocodone side.

I'll be honest though, this has never actually been reported in people, so it's a theoretical concern based on test-tube and animal data. The pseudoephedrine part isn't really affected. Given Fo-ti's separate link to liver injury, I'd run this past your pharmacist or doctor before pairing them.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's the gist with this combo cough and cold medicine. Your body uses a liver enzyme called CYP3A4 to help break down the hydrocodone part. Lab tests hint that Fo-ti might slow that enzyme down, which in theory could let hydrocodone build up a bit more than usual. If that happened, you might notice stronger opioid effects like extra drowsiness, dizziness, constipation, or slowed breathing.

I want to be honest with you, though: this is based only on test-tube and animal studies, the results even conflict, and it has never actually been reported in people. So I wouldn't panic. Still, since hydrocodone is a strong medicine, it's worth a quick chat with your pharmacist or doctor before mixing in Fo-ti.
