Perampanel falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2C19.
Animal and in vitro research suggests that fo-ti may inhibit CYP2C19. An in vitro study suggests that the degree of CYP2C19 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this interaction has not been reported in humans.

Here's the gist with Fycompa (perampanel) and Fo-ti. Your body uses several liver enzymes to clear perampanel, and one of them is CYP2C19. Lab and animal studies hint that Fo-ti may slow that enzyme down. If it does, perampanel could clear a bit slower and build up in your system. Perampanel is not a prodrug, so more drug hanging around generally means stronger effects and more side effects, like drowsiness, dizziness, trouble walking, or mood and irritability changes.

I'd keep this in perspective, though. This interaction has only been seen in test tubes and animals, never confirmed in people, so it's theoretical for now. Interestingly, raw Fo-ti seems to block the enzyme more than heavily processed extracts. Before combining them, check with your pharmacist or neurologist, since seizure meds deserve a careful eye.

Theoretically, fo-ti might increase or decrease the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.

Here's the gist with Fycompa (perampanel) and Fo-ti. Perampanel is partly cleared by a liver enzyme called CYP1A2, and Fo-ti may nudge that enzyme one way or the other. If Fo-ti slows the enzyme down, perampanel could build up a bit, which might mean more dizziness, sleepiness, trouble with balance, or mood and irritability changes. If it speeds the enzyme up instead, perampanel levels could drop and seizure control might slip.

The honest part: this is all from lab and animal studies, not real people, and the direction even flipped depending on the type of Fo-ti extract. So I wouldn't panic, but since steady seizure control matters so much, please run it by your pharmacist or neurologist before combining them.

Here's the gist for Fycompa (perampanel): your body clears it mainly through a liver enzyme called CYP3A4. Lab studies hint that Fo-ti, especially the raw root, might slow that enzyme down. If that happened in a person, perampanel could build up a bit and hit harder than expected. With this drug, that usually shows up as dizziness, drowsiness, trouble with balance or coordination, and irritability or mood changes.

I want to be honest about the strength of this, though. This is theoretical, drawn from test-tube work, and animal data actually disagree. It has never been reported in real people. So it's worth a quick mention to your pharmacist or neurologist, not a reason to panic.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
