Why this applies to Riociguat

Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's the gist for Adempas (riociguat): your liver uses an enzyme called CYP3A4 to help clear this medicine out of your body. Lab studies hint that Fo-ti might slow that enzyme down, especially the raw root. If that happened, riociguat could build up a bit more than expected, and since riociguat works by relaxing blood vessels, the main worry would be your blood pressure dropping too low. That can show up as dizziness, lightheadedness, headache, or feeling faint when you stand.

I want to be honest about the evidence though. This is only a theoretical concern from cell studies, animal data didn't agree, and it has never been reported in a real person. Still, riociguat is potent, so please run any Fo-ti by your pharmacist or doctor first.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).

The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
