Garlic and Iloprost

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Garlic interacts with whole categories of medication. Iloprost falls into 2 of those categories, so it can interact with Garlic in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

ANTIHYPERTENSIVE DRUGS

Why this applies to Iloprost

Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group Garlic interacts with.

Theoretically, taking garlic with antihypertensive drugs might increase the risk of hypotension.
In human research, both garlic and garlic extracts have blood pressure-lowering effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with iloprost (Ventavis): it's a vasodilator, meaning it works by relaxing and widening your blood vessels. Garlic, especially in concentrated supplement form, can also nudge blood pressure downward a bit. Put the two together and those effects can stack, so in theory you could feel more lightheaded, dizzy, flushed, or faint than usual, particularly when standing up quickly.

I want to be honest about the strength of this one. It's based on how these two work on paper, not on solid studies, and garlic's blood pressure effect is usually small. Cooking with garlic in your food is very unlikely to cause trouble. If you're thinking about garlic capsules or extracts, just run it past your pharmacist or doctor first so they can keep an eye on your numbers.

ANTICOAGULANT/ANTIPLATELET DRUGS

Iloprost also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Garlic may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Raw garlic and a variety of garlic extracts have antiplatelet activity and can increase prothrombin time.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about iloprost (Ventavis): besides relaxing the blood vessels in your lungs, it also makes your platelets a little less sticky, so it gently slows blood clotting on its own. Concentrated garlic supplements can do something similar, lowering how well your platelets clump together. Put the two together and you may nudge up the chance of bleeding or bruising more easily, longer bleeding from small cuts, nosebleeds, or bleeding gums.

To be fair, this is a theoretical concern based on how each one works, not strong proof, and normal garlic in cooking is very unlikely to matter. It's mostly the higher-dose garlic pills to watch. Before starting any garlic supplement, please check with your pharmacist or doctor first.

About Garlic

What is Garlic?

Garlic
  • High blood pressure
  • High cholesterol
  • Heart health
  • Immune support and common cold
  • Antimicrobial uses

Garlic is a common food and supplement that may modestly help with blood pressure and cholesterol, though the evidence is mixed and effects are usually small. It is generally safe in food amounts, but concentrated supplements can cause stomach upset, breath and body odor, and a higher risk of bleeding. Talk with your pharmacist or doctor before taking garlic supplements, especially if you take other medicines.
