Why this applies to Aluminum Hydroxide

Aluminum Hydroxide falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the simple version: glucomannan is a thick, gel-forming fiber, and when it's sitting in your stomach at the same time as an antacid like aluminum hydroxide, that gel can physically coat and trap some of the medicine. The worry is that less of your antacid gets absorbed or reaches the spot it needs to act, so you might get weaker relief from your heartburn, sour stomach, or ulcer pain.

This isn't a dangerous reaction, just a timing issue, and the evidence is fairly limited (mostly studied with other drugs, not aluminum hydroxide directly). An easy fix is to separate them. A good rule is to take your aluminum hydroxide first, then wait about 30 to 60 minutes before your glucomannan. If you take these regularly, run the timing by your pharmacist.

Aluminum hydroxide is used in two distinct ways depending on the product. The oral liquid form relieves heartburn, acid indigestion, and sour stomach.
