Here's the simple version: coffee charcoal has a porous, sponge-like surface, so when it sits in your stomach alongside aluminum hydroxide, it may grab onto some of that antacid before your body can use it. The likely result is that your antacid works less well, so you might not get the full relief from heartburn, sour stomach, or ulcer pain you were counting on.

This is a theoretical concern based on how charcoal behaves rather than hard clinical proof, so don't panic. The easy fix is timing. Keeping these two at least two hours apart usually sidesteps the problem. If you use both regularly, check with your pharmacist about the best schedule for you.
