Here's the simple idea: coffee charcoal acts like a sponge in your gut. Because of its porous, charcoal-like surface, it can grab onto medicine you swallow before your body absorbs it. With oral nitroglycerin (like the longer-acting capsules or tablets you swallow), that could mean less of the drug actually gets into your bloodstream, so it may not protect against chest pain as well as it should.

This is a theory based on how charcoal behaves, not something proven with nitroglycerin specifically, so don't panic. A good habit is to separate them by at least two hours. Important note: many nitroglycerin products are placed under the tongue or on the skin, and those aren't affected the same way. Check with your pharmacist about your exact product before changing anything.

What Is Nitroglycerin Topical Used For?

Brand names: Nitrostat, Nitrogard SR, Sustachron, Nitro-Bid, Sustachron ER Buccal, Nitroglyn, Nitro Time, Nitrong, Nitrogard, Nitrocine Timecaps, Nitroquick, Rectiv, Gonitro

Rectiv (nitroglycerin ointment 0.4%, applied rectally) is specifically indicated to treat moderate to severe pain associated with chronic anal fissure — a small tear in the lining of the anal canal.

Nitro-Bid (nitroglycerin ointment, applied to the skin) has been used to reduce exercise-related angina (chest pain brought on by physical activity). These are distinct products with separate uses; they are not interchangeable.
