Why this applies to Cedazuridine, Decitabine

Cedazuridine, Decitabine falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the concern with Inqovi (the decitabine and cedazuridine combo) taken by mouth: glucomannan is a thick, gel-forming fiber, and when it swells in your gut it can physically trap part of an oral medicine and slow or reduce how much gets absorbed. This is mostly a theoretical worry backed by a small human study with a different drug and some animal data, so it's not proven for Inqovi specifically. Still, this is a cancer treatment where getting the full dose matters, and absorbing less could mean it works less effectively against your myelodysplastic syndrome or CMML.

Because Inqovi is so important, I'd play it safe. Please check with your oncology pharmacist or doctor before using glucomannan. If they're okay with it, separating them (taking the fiber well after your medicine) helps reduce any overlap.

What Is Decitabine and Cedazuridine Used For?

Brand name: Inqovi

Inqovi is used to treat adult patients with myelodysplastic syndromes (MDS) — a group of blood cancers where bone marrow doesn't make enough healthy blood cells — and chronic myelomonocytic leukemia (CMML). It covers several subtypes of MDS, including refractory anemia, refractory anemia with ringed sideroblasts, refractory anemia with excess blasts, and both previously treated and newly diagnosed disease.

Inqovi is also approved in combination with venetoclax for the treatment of newly diagnosed acute myeloid leukemia (AML) in adults who are 75 years of age or older, or in adults of any age whose other health conditions make intensive chemotherapy unsafe.
