Why this applies to Ruxolitinib

Ruxolitinib falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the gist with ruxolitinib (Jakafi) taken by mouth: glucomannan is a thick, gel-forming fiber, and the worry is that it can physically trap part of a swallowed pill in your gut and slow or block how much gets absorbed. We have seen this with another oral drug (glyburide) in a small study, so for ruxolitinib it's a reasonable concern, though we don't have direct testing on this exact combo.

Ruxolitinib is not a prodrug, so the issue here is simply getting less of it into your bloodstream. Since it treats serious conditions like myelofibrosis, polycythemia vera, and GVHD, getting a lower dose than intended could mean it works less well. A simple fix is timing: take your ruxolitinib first, then wait 30 to 60 minutes before the glucomannan. Please run this by your pharmacist or oncologist before combining them.

Glucomannan is a soluble fiber from the konjac root that swells into a gel in your stomach, which may help with mild weight loss, constipation, and modestly lowering cholesterol. It is generally well tolerated when taken with plenty of water, but it must be taken carefully because it can swell and cause choking or blockage if not swallowed properly. What Is Ruxolitinib Used For?

Brand names: Jakafi, Opzelura

Ruxolitinib is approved to treat several serious blood and immune conditions in adults, and some in patients as young as 12. It is used for myelofibrosis (a bone marrow disorder that disrupts normal blood cell production), polycythemia vera (a condition where the bone marrow makes too many red blood cells), and two forms of graft-versus-host disease (GVHD) — a complication that can occur after a stem cell transplant.

For polycythemia vera, it is specifically for patients who have not responded well to, or cannot tolerate, a medication called hydroxyurea. For chronic GVHD, it is used after at least one or two other treatments have failed.
